Executive Summary An ageing population creates considerable challenges for the NHS and Social Care. A common news headline is ‘bed-blocking’ by the elderly because of the lack of suitable care- packages in place to allow them to leave hospital despite being capable of release on health grounds. Simplyhealth were approached by RVS to fund the introduction of a Home from Hospital Befriending Service at Swindon Hospital. The aim is to support the elderly at the time or release and for a number of weeks reducing the likelihood of early readmission. We committed a £40K grant in September 2015 to implement the programme. In March 2017, we provided a further £50K to fund a year’s extension and an enhancement to the programme with ‘Discharge bags’ of essential overnight provisions until an RVS volunteer can bring fresh supplies the following day. The extended funding enabled the local RVS administration team to compile quality evidence to build a business case for more sustainable funding. The local authority, convinced of the value of this service in keeping down hospital readmissions, has approved 3 years of funding from July 2018. This frees Simplyhealth to consider fresh project funding proposals from RVS.
